*Please note: This is a supplemental form to our standard registration packet and fees. If your school ager is new to our center, you will need to complete the Registration Packet, the Summer Camp Registration Form, and pay all required entrance fees. Thank you!
The Discovery Center’s Summer Camp Registration Form
Believe it or not, summer is right around the corner! The Discovery Center will provide day camp for school age children (6 years to 11 years) June 18, 2008 – August 26, 2008. This summer, camps will be broken into two-week intervals with a fun theme to explore for each two-week session. Field trips are already being planned – at least two per week! As a courtesy service to our customers, we allow you to sign up for the weeks that your child needs care, and bill only for the indicated time. This allows you to plan your family vacations, trips to Grandma’s, etc. without having to pay for unused childcare.
Tuition will be billed for the weeks you indicate, and are non-refundable.
**All field trip fees and meals are included in the price!
**There will be a Summer Camp Project fee of $75.00 due or billed to your account with this registration form. The fee includes your camper’s t-shirt, supplies, and supplemental gas expenses.

The Discovery's Center's 10% family discount will be honored for full time families.
**There will be a Summer Camp Project Fee for each camper due or billed to your account with this registration from. The fee includes your camper's t-shirt (2 for full time campers!) supplies, and supplemental gas expense. Fees are as follows:
$30 for full time campers $20 for 3/wk campers $15 for 2/wk campers
